Dear Mr. Chu,
Since there was no appeal to or suspension by the City Council, the March 24, 2003 Planning Commission
approval of your application for design review became effective Apri17, 2003. This application was to allow the
construction of a first and second story addition at 215 Humboldt Road, zoned R-l.
The March 24, 2003 minutes of the Planning Commission state your application was approved with the following
conditions:
1. that the project shall be built as shown on the plans submitted to the Planning Department date stamped
March 12, 2003, Sheets A1 through A4, site plan, floor plans, and building elevations, with the change
that the double hung window in the master bedroom closet on the front fa�ade shall be changed to a 2'-6"
x 2'-6" square divided light window, and shall have no shutters;
2. that any increase to the habitable basement floor area and any changes to the size or envelope of the first
or second floors, which would include expanding the footprint or floor area of the structure, replacing or
relocating a window (s), adding a dormer (s) or changing the roof height or pitch, shall be subject to
design review;
3. that the conditions of the City Engineer, Recycling Specialist and Fire Marshal's memos dated February
18, 2003 shall be met;
4. that prior to scheduling the roof deck inspection, a licensed surveyor shall shoot the height of the roof
ridge and provide certification of that height;

5. that prior to scheduling the framing inspection, the project architect, engineer or other licensed
professional shall provide architectural certification that the architectural details such as window locations
and bays are built as shown on the approved plans; if there is no licensed professional involved in the
project, the property owner or contractor shall provide the certification under penalty of perjury;
6. that prior to final inspection, Planning Department staff will inspect and note compliance of the
architectural details (trim materials, window type, etc.) to verify that the project has been built according
to the approved Planning and Building plans;
7. that the project shall meet all the requirements of the California Building and Fire Codes, 2001 edition, as
amended by the City of Burlingame;
8. that the applicant shall consult with the City Arborist to choose a larger size and scale tree from the City's
street tree list to replace the proposed Japanese maple trees at the rear of the lot; and
9. that an arborist report shall be prepared to determine the necessary protection measures for the existing
birch tree at the front of the lot and that it shall be protected in accordance with the report throughout the
construction period and with adequate fencing during construction.
Reimbursement of the design review deposit has been processed and will be mailed to the property owner under
separate cover.
All site improvements and construction work will require separate application to the Building Department. This
approval is valid for one year during which time a building permit must be issued. One extension of up to one
year may be considered by the Planning Commission if application is made before the end of the first year.
The decision of the Council is a final administrative decision pursuant to Code of Civil Procedure Section
1094.6. If you wish to challenge the decision in a court of competent jurisdiction, you must do so within 90 days
of the date of the decision unless a shorter time is required pursuant to state or federal law.
