In urbanized areas, up to three single-family residences maybe constructed or converted under this exemption, is hereby approved. 2. Said Design Review Amendment is approved subject to the conditions set forth in Exhibit "A" attached hereto. Findings for such Design Review Amendment are set forth in the staff report, minutes, and recording of said meeting. 3. Compliance with all conditions of approval is required; the conditions of approval shall not be modified or changed without the approval of the Planning Commission, or City Council on appeal; 8. that all air ducts, plumbing vents, and flues shall be combined, where possible, to a single termination and installed on the portions of the roof not visible from the street; and that these venting details shall be included and approved in the construction plans before a Building permit is issued; that the project shall comply with the Construction and Demolition Debris Recycling Ordinance which requires affected demolition, new construction and alteration projects to submit a Waste Reduction plan and meet recycling requirements; any partial or full demolition of a structure, interior or exterior, shall require a demolition permit; � /:11;3�ii�_VI Conditions of Approval for Categorical Exemption and Design Review Amendment 2747 Burlingview Drive Effective September 18, 2014 Page 2 9. that during demolition of the existing residence, site preparation and construction of the new residence, the applicant shall use all applicable "best management practices" as identified in Burlingame's Storm Water Ordinance, to prevent erosion and off-site sedimentation of storm water runoff; 10. that the project shall meet all the requirements of the California Building and Uniform Fire Codes, 2010 Edition, as amended by the City of Burlingame; THE FOLLOWING CONDITIONS SHALL BE MET DURING THE BUILDING INSPECTION PROCESS PRIOR TO THE INSPECTIONS NOTED IN EACH CONDITION: 11. that prior to scheduling the framing inspection the applicant shall provide a certification by the project architect or residential designer, or another architect or residential design professional, that demonstrates that the project falls at or below the maximum approved floor area ratio for the property; 12. that prior to scheduling the foundation inspection, a licensed surveyor shall locate the property corners, set the building footprint and certify the first floor elevation of the new structure(s) based on the elevation at the top of the form boards per the approved plans; this survey shall be accepted by the City Engineer; 13. that prior to scheduling the framing inspection the project architect or residential designer, or another architect or residential design professional, shall provide an architectural certification that the architectural details shown in the approved design which should be evident at framing, such as window locations and bays, are built as shown on the approved plans; architectural certification documenting framing compliance with approved design shall be submitted to the Building Division before the final framing inspection shall be scheduled; 14. that prior to scheduling the roof deck inspection, a licensed surveyor shall shoot the height of the roof ridge and provide certification of that height to the Building Division; and 15. that prior to final inspection, Planning Division staff will inspect and note compliance of the architectural details (trim materials, window type, etc.) to verify that the project has been built according to the approved Planning and Building plans. -... : ,... ,.., r �-� - - . _. ,: -.:, i . We apologize for the confusion generated from the previous product submitted, as it was our intention for it to represent only the turf blade material since it was the only product we had available. The proposed EasyTurf product is used in residential applications and doesn't suffer from the issue of tracking the ftll from the turf products, as in the commercial athletic products. We have installed this product on other residential projects (see photos attached), and we are familiar with the installation and results. The proposed product has a sand infill at the base, with an approximately 3/4" exposed blade. 2. A new home is planned for this site and your con.cern as to the future health and safety of the trees as well as your neighbors trees has prompted this visit. Method: All inspections were made from the ground; the tree was not climbed far this inspection. The tree in question was located on a"Not- to-Scale" map provided by me. The lree was then measured for dianaeter at 54 inches above ground level (DBH or diarneter at breast height). The tree was giveu a condition rating for form and vitality. The trees' condition ratzng is based on 50 percent vitality and 50 percent form, using the following scale. 1 - 29 Very Poor 30 - 49 Poor 50 - 69 Fair 70 - 89 Good 90 - 100 Excellent The height of the tree was measured using a Nikon Forestry 550 Hypsometer. Reducing the basement by six feet and using a mat slab type foundation near the sequoia will reduce root loss lessen irnpacts ta this tree. The following tree protection plans designed for this site should �e catxied out during the buiiding of this house. T�ee Protection Plan: Tree pratectioz� zones should be installed and maintained throughout the entire length of khe project. Fencing for tree protection should be 4' tall, orange plastic rnaterial supported by metal poles or stakes pounded into the ground. The location for the protective fencing should be as ciose to the dripline of desired trees as possible, still allowing room far construction to safely continue. No equipment or materials shall be stored or cleaned inside the protecrion zones. Areas outside protection %nce, but still beneath the tree's dripiine.s, where foot traffic is expected to be heAvy, should be mulched with 4-6" of chipper chips covered with plywood. The spreading of chips will help to reduce compaction arad improve soil struciure. Root Cutting Any roots to be cut shall be nionitored and documented. Large raots (over 2" diameter) or large masses of roots to be cut must be inspec;ted by the site arborist. The site arborist, at this time, may recammend irrigatton or fertilization of the root zone. All roots needing to be cut should be cut clean wzth a saw or lopper. Roots to be left exposed for a period of time should be covered with layers of burlap and kept moist. Tk�e over dig for the foundation should be reduced a much as possible when roots are eneountered. Hand digging and the careful placement of pipes below or besides protected roots will significanfly reduce root loss, thus reducing trauma to the tree. All trenches shall be backfiiled with native materials and compacted to near its original level, as soon as possible. Trenches to be left open for a period of time (24 hours), will require the covering of all exposed roots with burlap and be kegi moist. The trenches will also need to be covered with plywood to help protect the exposed roots. Irrigation The Neighbors trees will require regular irrigation. The trees should be irrigated 2 tirnes a month using heavy irrigation with enoagh water to wet the enlire root zone. Tree #1 shall be fertiiized using 300 gallons af 22-24-24 2 weeks prior to the start of excavation. Further irrigakion will be recommended if nceded hy the site arborist at the time of excavation. Compliance with all conditions of approval is required; the conditions of approval shall not be modified or changed without the approval of the Planning Commission, or City Council on appeal; 8. that all air ducts, plumbing vents, and flues shall be combined, where possible, to a single termination and installed on the portions of the roof not visibls frorn the street; and that these venting details shall be included and approved in the construction plans before a Building permit is issued; that the project shali comply with the Construction and Demolition Debris Recycling Ordinance which requires affected demolifion, new construction and alteration projects to submit a Waste Reduction plan and meet recycling requirements; any partial or full demolition of a structure, interior or exterior, shall require a demolition permit; ; ,._ _.. _ >.;: , .:...,_. , _. . . _ _ ... EXHIBIT "A" Conditions of Approval for Categorical Exemption and Design Review Amendment 2747 Burlingview Drive Effective September 18, 2014 Page 2 9. that during demolition of the existing residence, site preparation and construction of the new residence, the applicant shall use all applicable "best management practices" as identified in Burlingame's Storm Water �rdinance, to prevent erosion and off-site sedirnentation of storm water runoff; 10. that the project shall meet all the requirements of the California Building and Uniform Fire Codes, 2010 Edition, as amended by the City of Burlingame; THE FOLLOWING CONDITIONS SHALL BE MET DURING THE BUILDING INSPECTION PROCESS PRIOR TO THE INSPECTIONS NOTED IN EACH CONDITION: 11. that prior to scheduling the framing inspection the applicant shall provide a certification by the project architect or residential designer, or another architect or residential design professional, that demonstrates that the project falls at or below the maximum approved floor area ratio for the property; 12. that prior to scheduling the foundation inspection, a licensed surveyor shall locate the property corners, set the building footprint and certi#y the first floor elevation af the new structure(s) based on the elevation at the top of the farm boards per the approveci plans; this survey shall be accepted by the City Engineer; 13. that prior to scheduling the framing inspection the project architect or residential designer, or another architect or residential design professional, shall provide an architectural certification that the architectural details shown in the approved design which should be evident at framing, such as window locations and bays, are built as shown on the approved plans; architectural certification documenting framing compliance with approved design shall be submitted to the Building Divisian before the final framing inspection shall be scheduled: 14. that prior to scheduling the roof deck inspection, a licensed surveyor shall shoot the height of the roof ridge and provide certification of that height to the Building Division; and 15. that prior to final inspection; Planning Division staff will inspect and note compliance of the architecturai details (trim materials, window type, etc.) to verify that the project has been built according to the approved Planning and Building plans. ,_ , --, ._ _ __ . ,,-,,. .. . _ •.::: -- .
