Since there was no appeal to or suspension by the City Council, the
September 27, 1993 Planning Commission approval of your special permit
application became effective October 5, 1993. This application was to
allow retail services and sales at 1350 Bayshore Highway, Suite 30,
zoned C-4.
The September 27, 1993 minutes of the Planning Commission state your
application was approved with the following conditions:
1. that the project shall be built as shown on the plans submitted to
the Planning Department and date stamped August 26, 1993 Sheet A-P
Lower Level Plan (Before), Sheet A-P Lower Level Plan (After) and
InVisions proposed floor plan (8-1/2" x li");
2. that the hair replacement business will be open 9:00 A.M. to 9:00
P.M. Monday through Thursday, and Friday and Saturday 9:00 A.M. to
5:00 P.M., with a maximum of eight (8) employees and 30 visitors
growing to 50 in five years; and
3. that the project shall meet all the requirements of the Uniform
Building and Uniform Fire Codes as amended by the City of
Burlingame.
All site improvements and construction work will require separate
application to the Building Department. This approval is valid for one

year during which time a building permit must be issued. One �xtension
of up to one year may be considered by the Planning Commission if
application is made before the end of the first year.

RESOLUTION APPROVING CATEGORICAL EXEMPTION AND SPECIAL PERMIT
RESOLVED, by the Planning Commission of the City of Burlingame that:
WHEREAS, a categorical exemption has been proposed and application
has been made for a special permit for retail services and sales
at 1350 Bayshore Highway Suite 30 (APN 026-113-500�; {property
owner: Northwestern Mutual Life 720 E. Wisconsin Ave. N 15 NE
Milwaukee, WI 53202 ); and .
WHEREAS, said matters were heard by the Planninq Commission of the
City of Burlingame on September 27, 1993 , at which time it reviewed
and considered the staff report and all other written materials and
testimony presented at said hearing; .;;`

NOW, THEREFORE, it is RESOLVED and DETERMINED by this Planning
Commission that:
1. On the basis of the Initial Study and the documents submitted
and reviewed, and comments received and addressed by this commission, it
is hereby found that there is no substantial evidence that the project
set ferth above wilJ. hav� a significar.t effz�t on the environrner,t, and a
categorical exemption, Sec. 15301, Existing Facilities, Class 1(a),
Interior or exterior alterations ..., is hers�y appr��ed.
2. Sai'd special permit is approved subject to the conditions set
forth in Exhibit "A" attached hereto. Findings for such special permit
are as set forth in the minutes and recording of said meeting.
3. It is further directed that a certified copy of this resolution
be recorded in the official records of the County of San Mateo.

6. SPECIAL PERMIT FOR RETAIL SERVICES AND SALES (CONSULTING, SALON
_ AND GENERAL ADMINISTRATION FOR NON-SURGICAL HAIR REPLACEMENT) AT
1350 BAYSHORE HIGHWAY, SUITE 30, ZONED C-4
Reference staff report, 9/27/93, with attachments. CP Monroe reviewed
details of the request, required findings. Three conditions were
suggested for consideration at the public hearing.
Chm. Deal opened the public hearing. Kathy Absher, General Manager,
R&B Commercial Management Company, was present representing the
applicant, Peter Meyer, InVisions. She noted one of the suggested
conditions was that the project be built as shown on the plans
submitted and said there might be some slight variations in these
plans. CP advised this would be no problem as long as there will not
be more square footage. Responding to Commission questions Ms. Absher
said the main part of this business would be retail service, other than
the sale of hair pieces she did not know what products might be sold.
There were no audience comments and the public hearing was closed.
C. Graham found in reviewing the list of tenants in this building that
the applicant would not infringe on any other tenant's rights nor
create traffic problems, she found the use was suitable for the area.
C. Graham moved for approval of the special permit by resolution with

the following conditions: (i) that the project shall be built as shown
on the plans submitted to the Planning Department and date stamped
August 26, 1993 Sheet A-P Lower Level Plan (Before), Sheet A-P Lower

in the parking and ci ulatiqn areas; measurement of apparent width;
space between the two�olu�rfis on the side and the building; State
lands which border the �te and are landscaped; Condition #9 which
refers to timing of cons��ction work, it was suggested the wording be
changed to say 'site p�epar�tion'.

Development Inc., was present�s they are proposing to develop two
destination rest rants, one Asi�i and one Japanese, with a bar lounge;
top quality foo and service in a�Remorable environment is necessary to
draw people t the area. Mr. Dougla� commented with the use of slides:
the site is 7 acres, shaded area `'�n two sides is landscaped State
lands, it i a mitigation since that %nd appears to be a part of the
site, an a'ctual setting for the proje"ct of almost five acres. He
pointed c�tit design elements of the proje�,t, water features, ponds and
falls, skylights and exterior deck are� first floor entry with

rooms, middle is kitchen area, right side a Ja�nese restaurant; second
