CALIFORNIA 940i0-3997 rnx (4i5) 342-8386 November 18, 1993 Japan Kumon Educational Institute 111 Anza Blvd., Suite 100 Burlingame, CA 94010 Dear Mr. Hirabayashi, Since there was no appeal to or suspension by the City Council, the November 8, 1993 Planning Commission approval of your special permit application became effective November 15, 1993. This application was to allow training/classes at 111 Anza Blvd., Suite 100, zoned M-1. The November 8, 1993 minutes of the Planning Commission state your application was approved with the following conditions: 1) that the project shall conform to the plans submitted to the Planning Department and date stamped October 6, 1993 Sheet 1, Parking Plan and Floor Plan; 2) that the second exit shall be installed prior to any more training sessions taking place at the site; 3) that the project shall meet all the requirements of the Uniform Building and Uniform Fire Codes as amended by the City of Burlingame; and 4) that all trainees be 18 years or older. Al1 site improvements and construction work will require separate application to the Building Department. This approval is valid for one year during which time a building permit must be issued. One extension of up to one year may be considered by the Planning Commission if application is made before the end of the first year.
