Dear Mr. Reyes,
Since there was no appeal to or suspension by the City Council, the June 24, 2002 Planning Commission
approval of your application for design review became effective July 1, 2002. This application was to allow a
second story addition at 148 Costa Rica Avenue, zoned R-1.
The June 24, 2002 minutes of the Planning Commission state that your application was approved with the
following conditions:
that the project shall be built as shown on the plans submitted to the Planning Department date stamped
May 21, 2002, Sheets A2 and A3, floor plans and roof plan, and sheets A1 and A4, site plan and building
elevations date stamped June 13, 2002;
2. that any changes to the size or envelope of the basement, first or second floors, which would include
adding or enlarging a dormer(s), moving or changing windows and architectural features or changing the
roof height or pitch, shall be subject to design review;
3. that the project shall comply with the Construction and Demolition Debris Recycling Ordinance which
requires affected demolition, new construction and alteration projects to submit a Waste Reduction Plan
and meet recycling requirements; any partial or full demolition of a structure, interior or exterior, shall
require a demolition permit;
4. that the conditions of the Chief Building Official's, Recycling SpecialisYs and the City Engineer's May
28, 2002 memos shall be met;
5. that prior to scheduling the roof deck inspection, a licensed surveyor shall shoot the height of the roof
ridge and provide certification of that height;
6. that prior to scheduling the framing inspection, the project architect, engineer or other licensed
professional shall provide architectural certification that the architectural details such as window
locations and bays are built as shown on the approved plans; if there is no licensed professional involved
in the project, the property owner or contractor shall provide the certification under penalty of perjury;

7. that prior to final inspection, Planning Department staff will inspect and note compliance of the
architectural details (trim materials, window type, etc.) to verify that the project has been built according
to the approved Planning and Building plans;
8. that the property shall be surveyed and the side setbacks as shown on the schematic plans confirmed on
the building plans before a building permit shall be issued; and
9. that the project shall meet all the requirements of the California Building and Fire Codes, 1998 edition,
as amended by the City of Burlingame.
Reimbursement of your design review deposit has been processed and will be mailed to the property owners
under separate cover.
All sit : improvements and construction work will require separate application to the Building Department. This
approval is valid for one year during which time a building permit must be issued. One extension of up to one
year may be considered by the Planning Commission if application is made before the end of the first year.
The decision of the Council is a final administrative decision pursuant to Code of Civil Procedure Section
1094.6. If you wish to challenge the decision in a court of competent jurisdiction, you must do so within 90 days
of the date of the decision unless a shorter time is required pursuant to state or federal law.
