(b) Quality Assurance: The results of testing made on the CCRL Concrete Proficiency Sample Program numbered 175/176 were not submitted by the laboratory within the time allowed by the program; therefore, the report issued to the laboratory did not included ratings. In addition, records of the laboratory's investigation and corrective action taken when receiving low proficiency sample ratings were not maintained by the laboratory as required by Section 9.2.5 of C1077. Curinq Facilities (C511-13): (c) Records for the evaluation of data from the temperature recorder did not include the name of the person perForming the evaluation as required by Section 5.2 of C511. Unbonded Caps (C1231-14): (d) The bearing surfaces of the retainers, used for testing four inch diameter specimens, were found not to be plane within 0.002 inch as prescribed in Section 5.3 of C1231. Unit Weiqht Apparatus 1C138-14): (e) Records showing that the unit weight measures had been calibrated annually were not available in the laboratory. (fl An Ohaus scale, serial number 06263886FE, was checked and found not to conform to the accuracy requirement of C138 when tested at 50 pounds. Air Content Apparatus (Volumetricl (C173-14): (g) Records showing the air meter and calibrated cup were verified annually were not available in the laboratory. Air Content Aonaratus (Pressurel fC231-141: (h) Records showing that the meter had been calibrated at three month intervals were not available in the laboratory. Procedures: {i) Unit Weight of Concrete (C138-14): After filling the measure, the top surFace of the concrete was not struck off as prescribed in Section 6.6 of C138. (j) Air Content (Volumetric Method) (C173-14): The rolling procedure used with this test was not repeated to determine that a stable condition in the level of the water column had been obtained. (k) Cylinder Measurements (C3�14a): The diameters of the cylinders were determined by using measurements taken at the ends of the specimens, rather than at midheight as specified in Section 6.2 of C39. In addition, it was understood that the diameter of cylinder was not determined at the minimum frequency required by Section 6.3 of C39. (I) Cylinder Checks for Unbonded Caps (C1231-14): The ends of the specimen were not checked for perpendicularity prior to testing as required by Section 6.1 of C1231. Re: ' 3016 Alcazar Drive — New Accessory Dwelling Unit Dear Mr. Mazza, COMMUNITY DEVELOPMENT DEPARTMENT Planning Division PH: (650) 558-7250 FAX: (650) 696-3790 The Planning Division has reviewed your request for a new secondary dwelling unit and determined that the proposed project, which includes a new detached accessory unit located forward of the rear 30% of the lot, meets the performance standards set forth in Burlingame Municipal Code Chapter 25.59 (Accessory Dwelling Units), including a Conditional Use Permit for the plate and glazed opening heights that was approved by the Planning Commission on October 10, 2017. The legal status of your new detached accessory dwelling unit at 3016 Alcazar Drive, Zoned R-1, is subject to the following conditions of approval: 1. that the project shall be built as shown on the plans submitted to the Planning Division date stamped September 26, 2017; including that there shall be a minimum of three new evergreen trees planted at the front, left side of the property; 2. that prior to issuance of a building permit for the new accessory dwelling unit, an agreement of restriction shall be filed with the County Recorder. The Agreement of ' Restriction shall state that the secondary dwelling unit shall not be sold separately from any part of the property on which it is located; that it is restricted to the standards specified in the Accessory Dwelling Unit municipal code provision 25.59; and that either the primary or accessory dwelling unit shall be occupied as the principal place of residence of the property owner; 3. that demolition or removal of the existing structures and any grading or earth moving on , the site shall not occur until a building permit has been issued and such site work shall I be required to comply with all the regulations of the Bay Area Air Quality Management I District; 4. that the project shall comply with the Construction and Demolition Debris Recycling Ordinance which requires affected demolition, new construction and alteration projects to submit a Waste Reduction plan and meet recycling requirements; any partial or full demolition of a structure, interior or exterior, shall require a demolition permit; 5. that the project shall meet all the requirements of the California Building and Uniform ' Fire Codes, 2016 Edition, as amended by the City of Burlingame; �� Register online to receive City of Burlingame e-mail updates at www.burlingame.org o� O,ctober 23, 2017 si�16 Alcazar Drive Page 2 6. that the secondary dwelling unit permit may be revoked on any one or more of the ' following grounds: ' a. The perFormance standards are not being met; or b. No owner of the subject property resides on the property; or c. The secondary dwelling unit is no longer used for residential purposes; or d. The parking required is no longer provided; or e. The primary single family dwelling on the site is purposely demolished; or ' f. For those secondary dwelling units approved with reduced parking because the ' owner has entered into an agreement for continued affordability of the unit, the agreement between the owner and the city regarding affordability of the secondary dwelling unit has been breached by the owner and covered parking ins not being provided. 7. that if the accessory structure is demolished or the envelope changed at a later date the Conditional Use Permit shall be void or shall be amended to reflect the changes; $. that any recycling containers, debris boxes or dumpsters for the construction project , shall be placed upon the private property, if feasible, as determined by the Community Development Director; � 9. that the project shall comply with the Construction and Demolition Debris Recycling ; Ordinance which requires affected demolition, new construction and alteration projects ' to submit a Waste Reduction plan and meet recycling requirements; any partial or full demolition of a structure, interior or exterior, shall require a demolition permit; and 10. that the project shall meet all the requirements of the California Building and Uniform Fire Codes, 2016 Edition, as amended by the City of Burlingame. This approval is valid for one year during which time a building permit must be issued. An extension of up to one year may be considered by the Community Development Director if application is made before the end of the first year. The decision of the Community Development Director is a final administrative decision pursuant to Code of Civil Procedure Section 1094.6. If you wish to challenge the decision in a court of competent jurisdiction, you must do so within 90 days of the date of the decision unless a shorter time is required pursuant to state or federal law.
