Your request for an Administrative Use Permit to alter an existing wireless communication facility (facility) at
1761 Adrian Road, zoned RR, was reviewed by property owners within 300 feet and the Community
Development Department — Planning Division.
Since there was no appeal, the Administrative Use Permit has been approved for an application which
includes modifying an existing wireless facility consisting of adding three (3) new panel antennas and
associated equipment behind an existing screen wal� on the roof of a one-story commercial building.
The Administrative Use Permit was approved with the following conditions of approval:

that the Administrative Use Permit for to alter the existing wireless communication facility shall be
valid for ten (10) years from the date of approval. At least one hundred twenty (120) days prior to
the expiration of the initial ten (10) year term, the applicant shall complete and submit a renewal
application to the Community Development Director;
that the project shall be built as shown on the plans submitted to the Planning Division date
stamped December 30, 2015, sheets T-1, GN-1, GN-2, GS-1, A-1 through A-5, D-1, E-1, G-1 and
G-2;
that the project shall meet all the requirements of the California Building and Uniform Fire Codes,
2013 Edition, as amended by the City of Burlingame;
4. that the wireless communication facility shall operate in conformance with all applicable
provisions of Chapter 25.77 of the Burlingame Municipal Code (Wireless Communications);
where any conflicts exist between the applicable provisions of that chapter and this approval, the
more restrictive provision shall apply;
5. that prior to issuance of a building permit for construction of the project, the project construction
plans shall be modified to include a cover sheet listing all conditions of approval for the
Administrative Use Permit; which shall remain a part of all sets of approved plans throughout the
construction process. Compliance with all conditions of approval is required; the conditions of
approval shall not be modified or changed without an amendment to the Administrative Use
Permit;

6. that the facility shall meet or exceed current standards and regulations of the FCC, the FAA, and
any other agency of the state or federal government with the authority to regulate wireless
communication facilities. If such standards and regulations are changed and are made applicable
to existing facilities, the owners of the facilities governed by this chapter shall bring such facilities
into compliance with such revised standards and regulations within six (6) months of the effective
date of such standards and regulations, unless a different compliance schedule is mandated by
the controlling state or federal agency. Failure to bring the facility into compliance with such
revised standards and regulations shall constitute grounds for the removal of the facilities at the
owner's expense, revocation of any permit or imposition of any other applicable penalty;
7. that the facility shall be constructed of graffiti-resistant materials and shall be painted a non-
reflective material consistent with the color scheme on the building;
8. that any exterior lighting on the facility should have a manual on/off switch and be contained on-
site;
9. that signage in, on or near the facility should be prohibited with the exception of warning and
informational signs, which shall be designed with minimal aesthetic impact;
10. that within forty-five (45) days of commencement of the facility operation, the applicant shall
provide verification by independent qualified experts that the RF (radio frequency) levels of the
facility complies with FCC regulations and with the City noise regulations;
11. that the applicant shall report to the City every five (5) years from the date of commencement of
the facility operation, a review of the condition of the facility, of the facility's compliance with
federal and state regulations and of the facility's compliance with the provisions of this chapter
and the conditions of approval. The applicant shall also provide updated contact information for
the owner and the applicant and verifiable confirmation information as to what carrier(s) are using
the facility;
12. that the applicant shali procure and maintain a City business license, contact information for the
applicant, for the agent responsible for maintenance of the facility and for emergency contact;
13. that the applicant shall either secure a bond, letter of credit or other similar financial assurance, in
a form acceptable to the City, for the removal of the facility in the event that its use is abandoned,
its operation is ceased or the approval is terminated;
14. that maintenance and repairs to facility shall be permitted provided that such maintenance and
repair does not enlarge or extend the facility structure or equipment enclosures or change the
number, type, dimensions, of the antenna or related equipment;
15. that current contact information of the person or entity responsible for maintaining and repairing
the facility shall be provided to and maintained by the Community Development Department;

16. that the facility shall be kept clean and free of graffiti, litter and debris. Lighting, fences, shields,
cabinets, and poles, shall be maintained in good repair and free of graffiti and other forms of
vandalism, and any damage from any cause, including degradation from wind and weather, shall
be repaired as soon as reasonably possible to minimize occurrences of dangerous conditions or
visual blight. Graffiti shall be removed from any facility as soon as practicable, and in no instance
more than two (2) business days from the time of notification by any person or entity;
17. that except for emergency repairs, testing and maintenance activities that will be audible beyond
the property line shall only occur between the hours of 8:00 a.m. and 6:00 p.m. on Monday
through Friday, excluding holidays;
18. that noise originating from any equipment shall be mitigated to the extent necessary to ensure
compliance with applicable noise limitations under the Burlingame Municipal Code;
19. that backup generators shall only be operated during periods of power outages or for testing
during a set period;
20. that the service provider shall notify the Community Development Director of the intent to vacate
a site at �east thirty (30) days prior to the vacation;
21. that if the facility site is not operated for a continuous period of twelve (12) months, the
Administrative Use Permit shall be deemed terminated unless before the end of the twelve (12)
month period:
(1) The Community Development Director has determined that the same operator resumed
operation; or
(2) The City has received an application to transfer the permit to another service provider.
22. that no later than ninety (90) days from the date the facility is determined to have ceased
operation or the Provider has notified the Community Development Director of the intent to vacate
the site, the owner of the wireless communication facilities or the owner of the property on which
the facility is sited shall remove all equipment and improvements associated with the use and
shall restore the site to its original condition as required by the Community Development Director.
The provider or owner may use any bond or other assurances provided by the operator to do so.
The owner or his or her agent shall provide written verification of the removal of the facility within
thirty (30) days of the date the removal is completed.
All site improvements and construction work will require separate application to the Building Department.
This approval is valid for one year from the date of this approval letter, during which time a
building permit must be issued. One extension of up to one year may be considered by the
Community Development Director if such a request is received at least 120-days in advance of the
expiration date.

provide a Bond guaranteeing the removal of the facility in the event that its use is
abandoned, its operation is ceased or the approval is terminated at said location.
NOW, THEREFORE, THE CONDITION OF THE ABOVE OBLIGATION IS SUCH,
That if Principal shall remove the facilitv in accordance with said Approval Conditions
of Administrative Use Permit, then this obligation is void, otherwise to remain in full
force and effect.
PROVIDED, HOWEVER, That:
1. It shall be a condition precedent to any right of recovery hereunder, that in
event of any default on the part of the Principal, a written statement of the
particular facts of such default shall be forwarded to the Surety, within sixty
(60) days of the occurrence of such default, delivered by registered mail to
Surety at its Home Office located at: One Tower Square, Hartford, CT 06183.
2. That no action, lawsuit or proceeding shall be had or maintained against the
Surety on this Bond unless the same be filed and properly served upon the
Surety within one year from the effective date of the cancellation of the Bond.
3. That no right of action shall accrue under this Bond to or for the use of a
person or entity other than the Obligee, and its successors and assigns.
4. This Bond shall become effective April 20, 2016.
5. This Bond shall continue in full force and effect until canceled by the Surety
by providing thirty (30) days written notice to the Obligee.
6. The liability of the Surety shall in no event exceed the aggregate penal sum of
the Bond penalty.
7. If any conflict or inconsistency exists between the Surety's obligation or
undertakings as described in the Bond and as described in the underlying
document, then the terms of the Bond shall prevail.

Your request for an Administrative Use Permit to alter an existing wireless communication facility (facility) at
1761 Adrian Road, zoned RR, was reviewed by property owners within 300 feet and the Community
Development Department — Planning Division.
Since there was no appeal, the Administrative Use Permit has been approved for an application which
includes modifying an existing wireless facility consisting of adding three (3) new panel antennas and
associated equipment behind an existing screen wall on the roof of a one-story commerciai building.
The Administrative Use Permit was approved with the following conditions of approval:

that the Administrative Use Permit for to alter the existing wireless communication facility shall be
valid for ten (10) years from the date of approval. At least one hundred twenty (120) days prior to
the expiration of the initial ten (10) year term, the applicant shall complete and submit a renewal
application to the Community Development Director;
that the project shall be built as shown on the plans submitted to the Planning Division date
stamped December 30, 2015, sheets T-1, GN-1, GN-2, GS-1, A-1 through A-5, D-1, E-1, G-1 and
G-2;
that the project shall meet all the requirements of the California Building and Uniform Fire Codes,
2013 Edition, as amended by the City of Burlingame;
4. that the wireless communication facility shall operate in conformance with all applicable
provisions of Chapter 25.77 of the Burlingame Municipal Code (Wireless Communications);
where any conflicts exist between the applicable provisions of that chapter and this approval, the
more restrictive provision shall apply;
5. that prior to issuance of a building permit for construction of the project, the project construction
plans shall be modified to include a cover sheet listing all conditions of approval for the
Administrative Use Permit; which shail remain a part of all sets of approved plans throughout the
construction process. Compliance with all conditions of approval is required; the conditions of
approval shall not be modified or changed without an amendment to the Administrative Use
Permit;

6. that the facility shall meet or exceed current standards and regulations of the FCC, the FAA, and
any other agency of the state or federal government with the authority to regulate wireless
communication facilities. If such standards and regulations are changed and are made applicable
to existing facilities, the owners of the facilities governed by this chapter shall bring such facilities
into compliance with such revised standards and regulations within six (6) months of the effective
date of such standards and regulations, unless a different compliance schedule is mandated by
the controlling state or federal agency. Failure to bring the facility into compliance with such
revised standards and regulations shall constitute grounds for the removal of the facilities at the
owner's expense, revocation of any permit or imposition of any other applicable penalty;
7. that the facility shall be constructed of graffiti-resistant materials and shall be painted a non-
reflective material consistent with the color scheme on the building;
8. that any exterior lighting on the facility should have a manual on/off switch and be contained on-
site;
9. that signage in, on or near the facility should be prohibited with the exception of warning and
informational signs, which shall be designed with minimal aesthetic impact;
10. that within forty-five (45) days of commencement of the facility operation, the applicant shall
provide verification by independent qualified experts that the RF (radio frequency) levels of the
facility complies with FCC regulations and with the City noise regulations;
11. that the applicant shall report to the City every five (5) years from the date of commencement of
the facility operation, a review of the condition of the facility, of the facility's compliance with
federal and state regulations and of the facility's compliance with the provisions of this chapter
and the conditions of approval. The applicant shall also provide updated contact information for
the owner and the applicant and verifiable confirmation information as to what carrier(s) are using
the facility;
12. that the applicant shall procure and maintain a City business license, contact information for the
applicant, for the agent responsible for maintenance of the facility and for emergency contact;
13. that the applicant shall either secure a bond, letter of credit or other similar financial assurance, in
a form acceptable to the City, for the removal of the facility in the event that its use is abandoned,
its operation is ceased or the approval is terminated;
14. that maintenance and repairs to facility shall be permitted provided that such maintenance and
repair does not enlarge or extend the facility structure or equipment enclosures or change the
number, type, dimensions, of the antenna or related equipment;
15. that current contact information of the person or entity responsible for maintaining and repairing
the facility shall be provided to and maintained by the Community Development Department;

16. that the facility shall be kept clean and free of graffiti, litter and debris. Lighting, fences, shields,
cabinets, and poles, shall be maintained in good repair and free of graffiti and other forms of
vandalism, and any damage from any cause, including degradation from wind and weather, shall
be repaired as soon as reasonably possible to minimize occurrences of dangerous conditions or
visual blight. Graffiti shall be removed from any facility as soon as practicable, and in no instance
more than two (2) business days from the time of notification by any person or entity;
17. that except for emergency repairs, testing and maintenance activities that will be audible beyond
the property line shall only occur between the hours of 8:00 a.m. and 6:00 p.m. on Monday
through Friday, excluding holidays;
18. that noise originating from any equipment shall be mitigated to the extent necessary to ensure
compliance with applicable noise limitations under the Burlingame Municipal Code;
19. that backup generators shall only be operated during periods of power outages or for testing
during a set period;
20. that the service provider shall notify the Community Development Director of the intent to vacate
a site at least thirty (30) days prior to the vacation;
21. that if the facility site is not operated for a continuous period of twelve (12) months, the
Administrative Use Permit shall be deemed terminated unless before the end of the twelve (12)
month period:
(1) The Community Development Director has determined that the same operator resumed
operation; or
(2) The City has received an application to transfer the permit to another service provider.
22. that no later than ninety (90) days from the date the facility is determined to have ceased
operation or the Provider has notified the Community Development Director of the intent to vacate
the site, the owner of the wireless communication facilities or the owner of the property on which
the facility is sited shall remove all equipment and improvements associated with the use and
shall restore the site to its original condition as required by the Community Development Director.
The provider or owner may use any bond or other assurances provided by the operator to do so.
The owner or his or her agent shall provide written verification of the removal of the facility within
thirty (30) days of the date the removal is completed.
All site improvements and construction work will require separate application to the Building Department.
This approval is valid for one year from the date of this approval letter, during which time a
buiiding permit must be issued. One extension of up to one year may be considered by the
Community Development Director if such a request is received at least 120-days in advance of the
expiration date.

top of the structure is 18'-0". T-Mobile's associated equipment, including (2) cabinets, are
located within an existing shelter at the site. All antennas are currently located behind a screen.

Re: 6409 Exemption Qualifications for 1761 Adrian Road
Section 6409 of the Federal Middle Class Tax Relief and Job Creation Act of 2012 requires that
all jurisdictions grant approval to modifications of existing wireless sites if "they do not
substantially change the physical dimensions" of the existing site. Below is a list of the
qualifications for exemption and how the site at 1761 Adrian Road remains within those, thus
qualifying for approval through Section 6409.
• For towers in the public rights of way and for all base stations, the modifications or
collocations do not increase the height of the tower or base station by more than
10% or 10 feet, whichever is greater.
T-Mobile's proposed modification will not be increasing the height of the existing site. The
proposed height will remain at 29'-0", the same as the existing.
• For towers outside the public rights of way, the modifications or collocations do not
protrude from the edge of the tower by twenty feet or more than the width of the
Tower (whichever of these two dimensions is greater) at the level where the
transmission equipment modifications are made; for towers in the public rights of
way, and for all base stations, the modifications or collocations do not protrude from
the edge of the structure more than 6 feet.
T-Mobile's proposed modification will not be protruding from the side of the building.
• The modifications or collocations do not involve the installation of more than the
standard number of equipment cabinets for the technology involved, not to exceed
four cabinets.
The site currently has (2) equipment cabinets and T-Mobile will not be adding any additional
cabinets.
• The modifications or collocations do not entail any excavation or deployment outside
of the tower or base station site.
The site is roof top mounted and will not require any excavation or deployment.
• The modifications or collocations do not defeat any existing concealment elements
of the tower or base station
T-Mobile is proposing to add the new antennas behind the existing screen. There will be no
changes made to the dimensions of the screen. No aesthetics will change.
As per the above requirements and T-Mobile's compliance of these, it is our belief that the
site at 1761 Adrian Road should be exempt through Section 6409 of the Federal Middle
Class Tax Relief and Job Creation Act of 2012.

the FCC's occupational limit). The Proposed site is in compliance with Federal
regulations regarding (radio frequency) RF Emissions.
EBI Consulting was directed to analyze the Proposed T-Mobile rooftop facility located at 1761 Adrian
Road in Burlingame, Arkansas for the purpose of determining whether the emissions from the Proposed
T-Mobile Antenna Installation located on this property are within specified federal limits. This report
contains a detailed summary of the RF EME analysis for the site.
All information used in this report was analyzed as a percentage of current Maximum Permissible
Exposure (% MPE) as listed in the FCC OET Bulletin 65 Edition 97-Oland ANSI/IEEE Std C95.1. The
FCC regulates Maximum Permissible Exposure in units of microwatts per square centimeter (µW/cm2).
The number of µW/cm� calculated at each sample point is called the power density. The exposure limit
for power density varies depending upon the frequencies being utilized. Wireless Carriers and Paging
Services use different frequency bands each with different exposure limits, therefore it is necessary to
report results and limits in terms of percent MPE rather than power density.
All results were compared to the FCC (Federal Communications Commission) radio frequency
exposure rules, 47 CFR 1.1307(b)(I) —(b)(3), to determine compliance with the Maximum Permissible
Exposure (MPE) limits for General Population/Uncontrolled environments as defined below.
General population/uncontrolled exposure limits apply to situations in which the general public may be
exposed or in which persons who are exposed as a consequence of their employment may not be made
fully aware of the potential for exposure or cannot exercise control over their exposure. Therefore,
members of the general public would always be considered under this category when exposure is not
employment related, for example, in the case of a telecommunications tower that exposes persons in a
nearby residential area.
Public exposure to radio frequencies is regulated and enforced in units of microwatts per square
centimeter (µW/cmz). The general population exposure limit for the 700 and 800 MHz Bands is 467

µW/cmz and 567 µW/cm2 respectively, and the general population exposure limit for the PCS and AWS
bands is 1000 µW/cm2. Because each carrier will be using different frequency bands, and each frequency
band has different exposure limits, it is necessary to report percent of MPE rather than power density.
Occupational/controlled exposure limits apply to situations in which persons are exposed as a
consequence of their employment and in which those persons who are exposed have been made fully
aware of the potential for exposure and can exercise control over their exposure.
Occupational/controlled exposure limits also apply where exposure is of a transient nature as a result of
incidental passage through a location where exposure levels may be above general
population/uncontrolled limits (see below), as long as the exposed person has been made fully aware of
the potential for exposure and can exercise control over his or her exposure by leaving the area or by
some other appropriate means.

CALCULATIONS
Calculations were done for the Proposed T-Mobile Wireless antenna rooftop facility located at 1761
Adrian Road in Burlingame, Arkansas using the equipment information listed below. All calculations were
performed per the specifications under FCC OET 65. Because of the short wavelength of PCS services,
the antennas require line-of-site paths for good propagation, and are typically installed a distance above
ground level. Antennas are constructed to concentrate energy towards the horizon, with as little energy
as possible scattered towards the ground or the sky. This design, combined with the low power of PCS
facilities, generally results in no possibility for exposure to approach Maximum Permissible Exposure
(MPE) levels, with the exception of in areas in the immediate vicinity of the antennas.
For all calculations, equipment was calculated using the following assumptions:
I) 2 GSM / UMTS channels (PCS Band — 1950 MHz) were considered for each sector of the
Proposed installation. The transmit power for these channels is 30 watts per channel.
2) 2 UMTS channels (AWS Band — 2100 MHz) were considered for each sector of the
Proposed installation. The transmit power for these channels is 30 watts per channel.
3) 2 LTE channels (AWS Band — 2100 MHz) were considered for each sector of the Proposed
installation. The transmit power for these channels is 60 watts per channel.
4) I LTE channel (700 MHz Band) was considered for each sector of the Proposed installation.
The transmit power for this channel is 30 watts.
5) All radios at the Proposed installation were considered to be running at full power and were
uncombined in their RF transmissions paths per carrier prescribed configuration. Per FCC
OET Bulletin No. 65 - Edition 97-01 recommendations to achieve the maximum anticipated
value at each sample point, all power levels emitting from the Proposed antenna installation
are increased by a factor of 2.56 to account for possible in-phase reflections from the
surrounding environment. This is rarely the case, and if so, is never continuous.
6) EBI has performed theoretical worst case modeling using RoofviewOO to estimate the
maximum potential power density from each antenna based on worst-case assumptions for
the number of antennas and power.
7) The Data for all T-Mobile antennas used in this analysis is shown below in Table I. Actual
antenna gains for each antenna were used per manufacturer's specifications.

Summary
All calculations performed for this analysis yielded results that were within the allowable limits for
exposure to RF Emissions. Based on predictive modeling, chere are no modeled exposures on any
accessible ground-level walking/working surface related to T-Mobile's equipment in the area that exceed
the FCC's occupational and/or general public exposure limits at this site. T-Mobile can bring this site
into compliance by posting the recommended signage per this report.
The anticipated maximum contribution from each sector of the T-Mobile facility is 12.6% of the
allowable FCC established general public limit (2.52% of the FCC occupational limit). This was
determined through calculations along a radial from each sector taking full power values into account as
well as actual vertical plane antenna gain values per the manufacturers supplied specifications for gain.
The anticipated maximum composite MPE value for this site is 12.6% of the allowable FCC established
general public limit (2.52% of the FCC occupational limit). This is based upon worst case modeling
performed on the ground taking emissions contributions from all carriers present into account. This
value will determine whether the site itself is in compliance with regards to electromagnetic emissions
or whether mitigation solutions may be required to bring the site into compliance.
FCC guidelines state that if a site is found to be out of compliance (over allowable thresholds), that
carriers over a 5% contribution to the composite value will require measures to bring the site into
compliance. For this facility, the composite values calculated were within the allowable 100% threshold
standard per the federal government.
EBI's modeling indicates that there are no areas on the walking/working surfaces at the ground level in
front of the T-Mobile antennas that may exceed the FCC standards for general population andJor
occupational exposure.
In order to alert any workers potentially accessing the site, a blue Notice sign and a yellow Guidelines
sign are recommended for installation at the access to the rooftop as depicted on the Signage Plan —
Appendix B.
