RESOLVED, by the Planning Commission of the City of Burlingame that:
WI-�REAS, a Categorical Exemption has been proposed and application has been made for a
conditional use nermit for health service counseline at 1731 Adrian Road uite 2 zoned M-1 Lizah
and Steven McLaughlin. propertv owners APN� 109-390-020;
WHEREAS, said matters were heard by the Planning Commission of the City of Burlingame on _
7une 28�1999, at which time it reviewed and considered the staff report and all other written materials and
testimony presented at said hearing;
NOW, THEREFORE, it is RESOLVED and DETERMINED by this Planning Commission that:
1. On the basis of the Initial Study and the documents submitted and reviewed, and comments
received and addressed by this commission, it is hereby found that there is no substantial evidence that the
project set forth above will have a significant effect on the environment, and Categorical Exemption, per
CEQA Section 15301 is hereby approved.
2. Said conditional use permit is approved, subject to the conditions set forth in Exhibit "A"
attached hereto. Findings for such conditional use permit are as set forth in the minutes and recording of
said meeting.
3. It is further directed that a certified copy of�his resolution b� ecor� in the o$"icial
records of the Count of Saa� Mateo. ��

Conditions of approval Categorical Exemption and Conditional Use Permit
1731 Adrian Road, Suite 2
effective July 6, 1999
that the project shall be built as shown on the plan submitted to the Planning Department
and date stamped May 10, 1999, sheets with floor plan, unit area table and site plan
provided by tract map for 1731 Adrian Road;
2. that the counselin� service may not be open for business except during the hours after 5:00
p.m. on weekdays and on Saturdays between the hours of 9:00 a.m. and 5:00 p.m., with no
more than 1 counselor and 2 clients on site at any one time; and
3. that any improvements for the use shall meet all California Building and Fire Codes, 1998
Edition as amended by the City of Burlingame.
