This application was for health services in an existing office space at 1731 Adrian Road, Suite 2, zoned M-1. The June 28, 1999 minutes of the Plaruung Commission state your application was approved with the following conditions: 1. that the project shall be built as shown on the plan submitted to the Planning Department and date stamped May 10, 1999, sheets with floor plan, unit area table and site plan provided by tract map for 1731 Adrian; 2. that the project shall be built as shown on the plan submitted to the Planning Department and date stamped May 10, 1999, sheets with floor plan, unit area table and site plan provided by tract map for 1731 Adrian; 3. that the counseling service may not be open for business except during the hours after 5:00 p.m. on weekdays and on Saturdays between the hours of 9:00 a.m. and 5:00 p.m., with no more than 1 counselor and 2 clients on site at any one time; and 4. that any improvements for the use shall meet all California Building and Fire Codes, 1998 Edition as amended by the City of Burlingame. All site improvements and construction work will require separate application to the Building Department. This approval is valid for one year during which time a building permit must be issued. One extension of up to one year may be considered by the Planning Commission if application is made before the end of the first year. �_� ' - July 7, 1999 .
