(a) Quality Management System

(b) Organization
Nonconformity
The organization chart presented did not show all operational personnel positions and names of employees
(Section 53e3). The position of laboratory technician was not ineluded.
(c) Technician Training and Evaluation
Nonconformities
Training records were not presented for August Smarkel indicating that training had been performed for the
following test methods (Section 5.53): C131; D1556; D2041; D2488; D2726; and D3203. A similar finding was
noted durine the previous assessment, Report 677R. This indicates that the previous action taken to resolve the
issue may not have been effective.
The training records presented for Evan Provost indicated that evaluations had not been performed for test method
D421(Section 5.5.3).
(d) Equipment
Nonconformities
The laboratory did not have a system in place for performing regular maintenance on all applicable equipment
(Section 6.1.4. I). The following documents were not presented: a list giving a general description of equipment
that requires maintenance, detailed writte� procedures for in-house maintenance activities, and records of equipment
maintenance.
Maintenance intervals, written maintenance procedures, and records of equipment maintenance were not presented
for mechanical Proctar rammers (D698, D1557) and mechanical sieve shakers (Section 6.1.4.1).
Observation

procedure used for each itern u�hen the procedures cannot be referenced rn applicable standards (Section 6.1.2.1).
(e) Equipment Calibration, Standardization, Check, and Maintenance Records
Nonconformities
Calibration, standardization, or check intervals were not specified for expansion index vertical loads (D4829), L.A.
machines and steel spheres (C131), and weighted foot assemb]ies (D2419) (Section 6.1.2.1).
Written calibration, standardization, or check procedures were not presented far the following equipment (Section
6.1.2.3): hydrometers (D422); expansion index verYical loads and molds (D4829); length measurement devices
(D2166, D4829); L.A. machines and steel spheres (C131); and weighted foot assemblies (D2419).
Calibration, standardization, ar check records were not presented for L.A. machines and steel spheres (C131), and
weighted foot assemblies (D2419) (Section 6.1.5).
