Acceptable documentation was presented for August Smarkel and Evan Provost. Records of any damage, malfunction, or repair on equipment that has been subjected to overloading or mishandling, gives suspect results, or has been shown to be defective were not presented (Section 7.4.8). The equipment records presented did not include all of the required information, including 1) date received and 2) the history of any damage / malfunction / repair (Section 7.4). The quality manual did not contain procedures for control and maintenance of documentation (Section 8.2.4). The quality manual and related documentation did not identify the agency's approved signatories (Section 8.2.6).
