(g) ASTM C1077 - Staudard Practice for Laboratories Testing Concrete and Concrete Aggregates Nonconformities Standard operating procedures, which include the transfer of aggregate samples from the field to the laboratory, were not presented (Section 9.2,1.2). A procedure for ensuring the competency of any extemal calibration company used was not presented (Section ] 0.1.5). The faboratory did not retain records of the subcontractor and vendor evaluations for Pacific Nuclear Calibration Company (Section 10.1.5). Documents were not presented to show that all of the Aggregate Laboratory Technicians meet the certification requirements of C107� (Aggregate). Acceptable documentation was presented for August Smarkel and Evan Provost. However, Barry Lotz was performing aggregate testing during the on-site assessment without conforming to the certification requirements of C1077 (Aggregate). (h) ASTM D3740 - Standard Practice for Agencies Testing Soil and Rock Nonconformities Documents were not presented to show that all of the Testing Technicians meet the certification requirements of D3740. Acceptable documentation was presented for August Smarkel. However, Evan Provost was performing soil testing during the on-site assessment without the conforming to the certification requirements of this standard. Records verifying the competency of Pacific Nuclear Calibration Company used by the agency were not presented (Section 10.2.6). (i) ASTM E329 - Standard Speci6cation for Agencies Testing Materials Used in Construction Nonconformities The Iaboratory has not examined the effect ofdefective equipment on previous tests (Section 7.21.1). Records of any damage, malfunction, or repair on equipment that has been subjected to overloading or mishandling, gives suspect results, or has been shown to be defective were not presented (Section 7.4.8). The equipment records presented did not include all of the required information, including 1) date received and 2) the history of any damage / malfunction / repair (Section 7.4). The quality manual did not contain procedures for control and maintenance of documentation (Section 8.2.4). The quality manual and related documentation did not identify the agency's approved signatories (Section 8.2.6).
