RESOLUTION AUTHORIZING EXECUTION OF AMENDMENT TO SERVICE AGREEMENT WITH GATES MCDONALD FOR WORKERS' COMPENSATION ADMINISTRATION CONTRACT RESOLVED, by the CITY COUNCIL of the City of Burlingame, California and this Council does hereby FIND, ORDER and DETERMINE as follows: 1. The public interest and convenience require execution of the agreement cited in the title above. 2. The City Manager be, and he is hereby, authorized to sign said agreement for and on behalf of the City of Burlingame. 3. Coleman, City of Burlingame 501 Primrose Road Burlingame, California Dear Jerry: City Attorney 94010 G43TS, MCDONALD 1330 BROADWAY OAKLAND, CALIFORNIA 94612 415/832-0301 June 24, 1985 RE: Service Fee - 1985-86 This will confirm our telephone conversation of June 24 relative to the upcoming contract renewal and fee increase, As we discussed, we find it necessary to increase the fee from $125 to $150 per claim effective July 1, 1985. The May Claims Experience Report reflects that the City generated 53 claims from July 1, 1984 through May 31, 1985. Annualized through the end of June 1985, we can probably expect from 58 to 60 claims. Therefore, I would recommend that quarterly instalments be decreased from $2,800 to $2,250, In that the City was billed for an estimated 90 claims this current fiscal year, you can ex- pect a substantial credit of from $3,500 to $4,000, I am taking the liberty of enclosing an original and three copies of an amendment to the contract reflecting the appropriate changes in the fee and term clauses. If acceptable to you and the City Council, I would appreciate your signing and returning two copies to me for our records. If you have any questions or require clarification, please call me.
