RESOLUTION APPROVING CATEGORICAL EXEMPTION
AND CONDITIONAL USE PERMIT
RESOLUTION-58-99
RESOLVED, by the Planning Commission of the City of Burlingame that:
WHEREAS, a categorical exemption has been proposed and application has been made for a
conditional use =mit for health service (chiropractic officel in an existing office building at 1220
Howard Avenue, zoned C-1, Subarea B, Dominick and Patti Crisafi, 5 Homs Court, Hillsborough, CA
94010, 12roperty owners, APN: 029-204-240;
WHEREAS, said matters were heard by the Planning Commission of the City of Burlingame on
May 24, 1999 , at which time it reviewed and considered the staff report and all other written materials
and testimony presented at said hearing;
NOW, THEREFORE, it is RESOLVED and DETERMINED by this Planning Commission that:
1 . On the basis of the Initial Study and the documents submitted and reviewed, and comments
received and addressed by this commission, it is hereby found that there is no substantial evidence that the
project set forth above will have a significant effect on the environment, and categorical exemption, per
CEQA Section 15301, existing facilities, Class 1(a), interior or exterior alterations involving such things
as interior partitions, plumbing, and electrical conveyances is hereby approved.
2. Said conditional use permit is approved, subject to the conditions set forth in Exhibit "A"
attached hereto. Findings for such conditional use permit are as set forth in the minutes and recording of
said meeting.
3. It is further directed that a certified copy of this resolution be recorded in the official records
of the County of San Mateo.

ta 1. that the chiropractic office shall be limited to 1064 SF on the first floor at 1220 Howard
,r-_ Avenue, as shown on the plans submitted to the Planning Department and date stamped

01"° 2. that the chiropractic office may not be open for business except during the hours of 8:00
'' a.m.to 6:00 p.m.,Monday through Saturday with a maximum of 4 people(employees and
clients)on site at any one time;
3. that any changes in operation,floor area,use, or number of employees,which exceeds the
maximums as stated in these conditions shall require an amendment to this use permit;and
4. that the use and any improvements for the use shall meet all California Building and Fire
Codes, 1995 Edition as amended by the City of Burlingame.
