On the basis of the Initial Study and the documents submitted and reviewed, and comments received and addressed by this commission, it is hereby found that there is no substantial evidence that the project set forth above will have a significant effect on the environment, and categorical exemption Section 15301 - Existing Facilities, Class 1'(a), Interior or exterior alterations involving such things as interior partitions, plumbing, and electrical conveyances, is hereby approved. 2. Said special permit and parking variance is approved subject to the conditions set forth in Exhibit "A" attached hereto. Findings for such Special Permit and Parking Variance are as set forth in the minutes and recording of said meeting. 3. It is further directed that a certified copy of this resolution be recorded in the official records of the County of San Mateo. I, RUTH E. JACOBS , Seretary of the Planning Commission of the City of Burlingame, do hereby certify that the foregoing resolution was introduced and adopted at a regular meeting of the Planning Commission held on the 14th day of March , 1994 , by the following vote: AYES: COMMISSIONERS: DEAL, ELLIS, GALLIGAN, KELLY AND MINK NOES: COMMISSIONERS: NONE ABSENT: COMMISSIONERS: GRAHAM SECRET Y EXHIBIT "A" Conditions of approval, categorical exemption, special permit and parking variance 1606 Rollins Road (effective March 21, 1994) 1. that the project shall be built as shown on the plans submitted to the Planning Department and date stamped January 27 , 1994 Sheet A-1, Site Plan and A-2 Existing and Proposed Floor Plan; 2 . that the conditions of the Chief Building Inspector's February 7, 1994 memo shall be met; 3 . that the parking lot shall be restriped to accommodate 24 parking stalls, including one disabled accessible, 15 standard, and 8 compact stalls; 4 . that the loading dock/service entrance for the ambulance business (1606 Rollins Road) shall only accept van type trucks and not tractor trailer vehicles; 5. that the office and supply/storage ambulance business will be open 6: 00 A.M. to 8: 00 P.M. Monday through Sunday with a maximum of 20 employees on site at any one time; 6. that the office and supply/storage ambulance business shall be reviewed for conformance with the conditions of approval within six months (October, 1994) o:^ upon compliant; 7. that the project shall meet all the requirements of the Uniform Building and Uniform fire Codes as amended by the City of Burlingame; and 8 . that the variance for 5 parking spaces and 59% office use shall be tied to a continuation of the ambulance service use of both 1616 Rollins Road and 1606 Rollins Road a change in this joint use of either site by such use shall void the use permit and parking variance.
