RESOLUTION APPROVING CATEGORICAL EXEMPTION
SPECIAL PERMIT AND PARKING VARIANCE
RESOLVED, by the Planning Commission of the City of Burlingame that:
WHEREAS, a categorical exemption has been proposed and application has been made for a
special permit to allow a health service use in 140 SF office of an existing office building and a one
car parking variance at 405 Primrose Road #206 zoned C-1 Subarea BI: APN: 029-121-450 ;
property owner: Terry Horn, 405 Primrose Road, Suite 300 ; and
WHEREAS, said matters were heard by the Planning Commission of the City of Burlingame on
September 11, 1995 , at which time it reviewed and considered the staff report and all other written
materials and testimony presented at said hearing;
NOW, THEREFORE, it is RESOLVED and DETERMINED by this Planning Commission that:
1. On the basis of the Initial Study and the documents submitted and reviewed, and comments
received and addressed by this commission, it is hereby found that there is no substantial evidence that
the project set forth above will have a significant effect on the environment, and categorical exemption
per Section 15301-Existing facilities, Class 1 (a), Interior or exterior alterations involving such things as
interior partitions, plumbing and electrical conveyances is hereby approved.
2. Said special permit and parking variance is approved subject to the conditions set forth in
Exhibit "A" attached hereto. Findings for such variance and Special Permit are as set forth in the
minutes and recording of said meeting.
3. It is further directed that a certified copy of this resolution be recorded in the official
records of the County of San Mateo.

Conditions of approval categorical exemption, special permit and parking variance
405 PRIMROSE ROAD,#206
effective SEPTEMBER 18,1995
1. that the 140 GSF area designated as Suite#206 shown on the floor plan submitted to the
Planning Department and date stamped August 1, 1995 shall be used as a health service
for a marriage, family,child counselor business;
2. that the marriage,family,child counselor business shall have a maximum of two
employees, including the practitioner, have office hours Monday through
Thursday, for a maximum of 28 hours each week and will be closed 12:00 Noon
to 2:00 p.m.,with appointments scheduled no closer than 10 to 15 minutes apart;
3. that this use permit and parking variance shall apply only to this 140 GSF office
area and shall not be transferred within the structure without amendment to this
use permit and that this use permit and parking variance shall expire should this
site be rented for 6 months or more to any other use;
4. that any change to the number of employees, size of Suite #206 or the area
occupied by this business,including leasing additional office area, any change in
the hours or days of operation shall require an amendment to this use permit;and
5. that any tenant improvements and use of the 140 GSF shall meet all the
requirements of the Uniform Building and Uniform Fire codes as amended by the
City of Burlingame.
