AUTHORIZING EXECUTION OF RENEWAL OF
CONTRACT WITH EMPLOYERS SELF-INSURANCE
COMPANY (ADDENDUM - ESIS CONTRACT ## 481)
RESOLVED, by the CITY COUNCIL of the City of Burlingame, California,
and this Council does hereby FIND, ORDER AND DETERMINE as follows:
1. The public interest and convenience require execution of the
agreement cited in the title above.
2. The City Manager be and he is hereby authorized to sign said
agreement for and on behalf of the City of Burlingame.
3. The City Clerk is hereby ordered and instructed to attest
such signature.

I, EVELYN H. HILL, City Clerk of the City of Burlingame, do
hereby certify that the foregoing Resolution was introduced at a regular
meeting of the City Council held on the 17 day of April , 1978 ,
and adopted thereafter by the following vote:

ESIS CONTRACT #481
In consideration of the fee charged, it is hereby understood and
agreed that this contract is renewed for the period July 1, 1978,
through June 30, 1980, under the same terms and conditions, but
with an agreed upon service fee of $4,000. for the first year,
and $4,000., plus $58. for each claim in excess of seventy (70)
for the second year.
IN WITNESS WHEREOF, the parties have caused this contract to be
executed by the persons authorized to act in their respective
names.
