Said special permit amendment is approved, subject to the conditions set forth in Exhibit "A" attached hereto. Findings for such special permit amendmcm areas set forth in the minutes and recording of said meeting. 3. MALFATTL City Clerk of the City of Burlingame, do hereby certify that the foregoing resolution was introduced at a regular meeting of the City Council held on the 18th day of May , 1998 , and adopted thereafter by the following vote: AYES: COUNCILMEMBERS: GALLIGAN, JANNEY, O'MAHONY, SPINELLI NOES: COUNCILMEMBERS: - KNIGHT ABSENT: COUNCILMEMBERS: NONE CITY CLERK EXHIBIT "A" Conditions of approval categorical exemption and special permit amendment 1108 BURLINGAME AVENUE effective May 18, 1998 1. that the food delivery service shall only be offered after 5:00 p.m. and there shall be a maximum of five part time employees associated with the food delivery service and any change to the number of employees shall require and amendment to this use permit; 2. that vehicles used for the food delivery service shall not park in any loading zone or on Burlingame Avenue while picking up delivery orders; 3. that the fire lane in the driveway at the rear of the site accessed from California Drive shall be maintained clear and that no vehicles used for the food delivery service shall be parked overnight in this driveway area; and 4. that this special permit amendment shall be reviewed in one year (April 1999) and each two years thereafter for compliance with these conditions and/or upon complaint. 7 hereby certify this to be a full, true and correct copy of the document it purports to be, the original of which is on file in my office.
